New York State Testing
HNL Genomics holds a New York State Department of Health Clinical Laboratory Permit to perform molecular genetic testing on patients from New York State.
Osteogenesis Imperfecta
Status: Osteogenesis Imperfecta tests have been approved and may be run on all New York state patients.
- Complete a Patient Consent form.
- Complete a Test Requisition Form.
- Submit both the patient consent form and the test requisition form to HNL Genomics with the patient specimen.
Approved Panels:
- Osteogenesis imperfecta COL1A1 & COL1A2 NGS panel
- Osteogenesis imperfecta core NGS panel
- Osteogenesis imperfecta Dominant NGS panel
- Osteogenesis imperfecta Recessive NGS panel
- Osteogenesis imperfecta Dominant & Recessive NGS panel
Please note that the MBTPS2 and XYLT2 genes are not included in the New York State approved test.
To perform any other test on a New York State patient, a Non-Permitted Laboratory Test Request Approval Form must be completed and submitted to the New York State Department of Health. If the request is approved, HNL Genomics can perform the test for that patient. Please submit a copy of the approval letter and the HNL Genomics standard Test Requisition Form with the patient specimen. To perform any other test on a New York State patient, a Non-permitted Laboratory Test Request Approval Form must be completed and submitted to the New York State Department of Health.
How to Order:
- Please Complete the approval form.
- Complete the HNL Genomics (CTGT) standard Test Requisition Form.
- Submit both the approval form and the test requisition form with the patient specimen.
