| Description | ||||||||||||||||||||||||||||||
|
The acrofacial dysostoses are characterized by malformations of the craniofacial skeleton and limbs. The major craniofacial findings in acrofacial dysostosis 1, Nager type (AFD1; MIM 154400) include downslanting palpebral fissures, lower lid coloboma, microcephaly and micrognathia with midface retrusion, cleft lip or palate, posteriorly rotated low set ears with conductive deafness and macrostomia. Upper limb abnormalities can include hypoplasia or aplasia of the radii and thumbs, syndactyly, clinodactyly, radioulnar synostosis, triphalangeal thumbs and phocomelia. Lower limb abnormalities including aplasia or hypoplasia of the toes have also been described. Although AFD1 may be genetically heterogeneous, a high percentage of cases have been defined with dominant mutations in splicing factor 3B, subunit 4 (SF3B4), a component of the U2 pre-mRNA spliceosomal complex. Although some AFD1 cases are inherited, most cases seem to be sporadic. |
