Clinical Utility

Evaluate muscle wasting process. High levels are found in progressive Duchenne muscular dystrophy (MD). Elevations occur in carriers of MD, in limb-girdle dystrophy and other dystrophies, in dermatomyositis, polymyositis, and trichinosis, but not in neurogenic atrophies (eg, multiple sclerosis or in myasthenia gravis).

Specimen and Container Info

Specimen Type: Serum

Alternate Specimen Type: Plasma

Collection Container: Gold Top, Serum Separator

Alternate Collection Container: Red Top, Plain, Lavender Top (EDTA), or Green Top (Sodium or Lithium Heparin)

Desired Specimen Volume: 0.5 mL

Collection Instructions
  1. Collect a gold top, serum separator tube via venipuncture.
Processing and Submission Instructions
  1. Separate serum from cells immediately after coagulation (30 minutes).
  2. Transfer 0.5 mL of serum to a clear aliquot tube (screw cap).
Transport Instructions

Transport to the laboratory refrigerated

Stability Requirements

Room Temperature

1 day

Refrigerated

7 days

Frozen

15 days

Freeze/Thaw Cycles

 Stable x3

Causes for Rejection
  • Hemolysis (red cells contain aldolase)
  • Gross lipemia
Methodology

Reference Intervals

See patient report.

Limitations

Not removing refrigerated specimens from the clot results in aldolase levels 12% to 46% higher.

As muscle mass diminishes, aldolase decreases. Serum aldolase elevation is not specific for muscle disease since it is present in many tissues
