Collection Instructions
Submit on sterile towel, sterile Petri dish, or empty sterile jar. Label with patient's full name and second patient identifier (i.e., DOB, MR#, SS#). Include specific specimen source and laterality, if applicable.
Processing and Submission Instructions
After normal hours, refer to On-Call Pathologist Schedule; call prior to specimen collection to allow for travel time. Submit specimen with a completed Intraoperative Pathology Consultation Requisition Form (LAB-38).
