Collection Instructions
  • Collect in plastic container or bag large enough to contain specimen.
  • Container/bag needs to be labeled with patient's full name and second patient identifier (i.e., DOB, MR#, SS#).
  • Include specific specimen source and laterality, if applicable.
Processing and Submission Instructions
  • Submit specimen with an EPIC order, Tissue Pathology Requisition Form (HNL-31), or one of the laboratory's appropriate electronic forms.
