Collection Instructions
Collect specimen in 100% ethyl alcohol. Do NOT place in formalin.
Processing and Submission Instructions
Submit specimen with an EPIC order, Tissue Pathology Requisition Form (HNL-31), or one of the laboratory's electronic forms. Include client history, source of specimen and requested testing.
Causes for Rejection
Specimen cannot be submitted in formalin (100% ethyl alcohol only.)
