Collection Instructions
Collect on sterile towel, sterile Petri dish, or in a sterile container. Specimen container or slides must be labeled with patient’s full name, date, time and a second identifier (i.e., DOB, MR#, SS#.)
Processing and Submission Instructions
Submit specimen with an EPIC order, Tissue Pathology Requisition Form (HNL-31), Cytology Non-Gyn Requisition (HNL-32), or one of the laboratory's electronic forms. Include patient history, source of specimen and requested testing. After normal hours, call laboratory for on-call pathologist prior to specimen collection (to allow for travel time.)
Causes for Rejection
Testing CANNOT be performed on tissue that has been fixed in formalin.
